1. The Cohen’s Lifestyle Centre (CLC) Singapore Holiday Voucher promotion will run from November 15 – December 31, 2017.
  2. This promotion is available at CLC Singapore branch only.
  3. This promotion is open to all individuals who will sign-up for the Cohen Program at the time of their info session orientation day.
  4. Enrollees who are eligible for the promotion will be entitled to one (1) Holiday Voucher worth up to SG$300 for the purchase of the required BLOOD TESTS needed for their Cohen Program in their first 6-month contract duration.
  5. A maximum of one (1) Initial Blood Test and four (4) Repeat Blood Tests can be purchased per individual as needed by the program.
  6. In the event an individual is deemed not suitable to undergo the program due to a medical condition and/or abnormal initial blood test results, as per Dr. Cohen’s evaluation, a full refund of the program cost will be given less the cost of the Initial Blood Test worth SG$100.
  7. The grace period for refund expires once the initial blood test results are submitted. No refund may be made once the initial blood test results are submitted to CLC Singapore.
  8. This Holiday Voucher may only be used for the purchase of the required blood tests needed for the Cohen Program.
  9. Any amount remaining/unused on the Holiday voucher cannot be converted to a cash refund.
  10. This promotion is non-transferrable and non-convertible to cash or other items.
